Opinion Outpost

I've only been with Opinion Outpost for less than a month and just requested my first $5 check.  They've sent several surveys that I was a little late on checking and were closed, but the ones I got around to doing were quick.  I highly recommend this site for extra cash.  You only need 50 points to cash out for a $5 check.
